The central twist is that Teddy Daniels is not actually a U.S. Marshal. His real name is Andrew Laeddis, a patient at Ashecliffe Hospital. Years earlier, Andrew's wife, Dolores, suffered from severe mental illness. Despite warning signs, Andrew failed to get her proper treatment. One day, she drowned their three children. In shock and grief, Andrew shot and killed her. Unable to cope with the trauma and his own guilt, Andrew created the fantasy that he was "Teddy Daniels," a heroic marshal investigating a conspiracy. In this fantasy: His wife was murdered by an arsonist. He is hunting that killer. The hospital is experimenting on patients with mind-control and brainwashing. The doctors, led by Dr. Cawley, allow Andrew to live out this elaborate role-play as a last attempt to break through his delusion. Everyone on the island—including "Chuck," who is actually Andrew's psychiatrist—is participating in the therapeutic experiment.
